Usually only happens while aimlessly sprinting + mis-timing a jump across some obstables - as should be. It happened maybe 3-4 times in total - which makes it at the most 1 per entire playthrough, so - it's RARE, but I like it

There was a hiking mod for FNV that did that. It was funny but hard to play with when running downhill.
