How hard do you make your playthroughs?

I'm doing another playthrough of this game, and I've got Project Nevada, hardcore, and a few other mods. And I've got the difficulty nearly maxed out. Holy crap.. This is just nuts. I just got past saving Deputy Beagle in Primm, and I nearly ripped my balls off in frustration. But it's still a lot of fun. Do you guys like to torture yourself too?

You should download the DUST mod and play it on very hard with hardcore mode on

After War Nevada upon Hard is already a trek to go through.
You see, instead of simply increasing the damage you take and hitpoints you enemies take, it also changes upon crucial stats, like weapon sway, recoil, action point regeneration and cost, movement speed, healing rate, so on.
So it's not just hard because you ended up hurting yourself more than ever, it's harder because you have harder time to cope up with reduced stats to aiming, VATS ect.
