I'm a purist, but don't mind certain changes that were obviously intended, such as the fixed Followers path. I would just use caution when veering into mod territory, as you might want to make it a side project. At the least, right now the installer makes it clear what options are customizable...

Bit hesitant on the idea of new features like playing as a super mutant or killing the overseer. There's patching and then there's modding. The Vampire: The Masquerade- Bloodlines Unofficial patch suffers HORRIBLY from this.

Many A True Nerd has a rather huge following, and is known for his Fallout 3, New Vegas, and Fallout 4 runs.

Basically, as long as there's a purist, bugfix only version, I'm good. I'm a bit hesitant about the switch to the new engine, but we'll see how it goes. This patch hasn't disappointed so far.
Are you sure you selected the right version to install those files? The 'fixes only' option won't add those in, you might want to re-install the patch and make sure those changes are selected.
Excellent! That just leaves the random typos and cases where subtitles didn't match speech. If I have time, and if you want, I can go through the entire script to hunt down anything left, but I think I listed the main ones that were notable.
