I run with jsawyer mod which you can get at his blog here,
http://twofoldsilence.diogenes-lamp.info/ and I run Mission Mojave Ultimate Edition , which is a huge bug fix patch...from what I have seen it doesn't really change any quets or such what though there are some minor appearnce changes made to various characters(nothing major, all vanilla assets) you can get it here

Apart from that I don't use much else.
My experience with pip boy replacers has been poor, I know a lot of people really like them but I found they make my game crash like it was going out of style.
Pipboy 2500 is solid. Never crashed for me before.
Enhance Blood Textures is great too.
EVE is amazing for those who like to use Energy weapons.
Project Nevada is probably the best overhaul available.
Missions Mojave caused me more problems than good when I used it for awhile and figured out what it changed. It broke several quests and added a bunch of glitches. It is a good mod but I am going to wait until they finish it now. They change some things that imo don't need to be changed.
Hmm..I'll just copy from the other thread.
Some of these have been mentioned already. Oh well.
Sawyers mod. Must have.
NVinteriors adds so much it would be a crime not to use it. I always wanted to explore those train cars and some of those boarded up building. Now you can!
DFB Random Encounters adds a few spawn points with enemies of your choosing. Enclave is off by default thank God. It's one of those mods that is essential for me, simply because I know where every single enemy is in the vanilla game, and it gets boring after awhile without some variety.
Try Performance of the Gods if your computer is real crappy. It will speed things up.

Darnified UI is a must, to get rid of those ugly ass fonts. Mtui is a nice alternative.
Pipboy 2500 is the perfect mix of nostalgia and improvements. Lots of skins for the readius mods too.
Centered 3rd person camera does just what it says. You can even zoom out and almost trick yourself into believing it is Fallout 2...almost.
Weapon Mods Expanded allows you to mod unique weapons, adds a few mods, and works quite well with Project Nevada. It never leaves my load order.
Personally, I like making my raider chicks look like wasteland sluts with genitalia piercings, titties hanging out, and other such perversions. For instance, it bothered me in Gomorrah how the whores didn't dress like whores. I fixed that. Just a minor thing that bugged me.
Bottle that Water mod is great too. It does what it says.
KDE Portable Camp Stuff is worth checking out too. It's hard to describe, but you can basically drop various things such as stairs, beds, walls, campfires, reloading benches, to make your own campsite or home.

I always use a backpack mod too because, well, it makes sense in the wasteland to have something to carry your shit in. Shit needs to be carried people. It doesn't magically disappear. Stupid game designers j/k...
Tfh Rugged Race. Make your men look like MEN. I just started a playthrough with this, and I will NEVER play without it again. You want to look like Clint Eastwood? Now you can.

Fake fullscreen. Improves stability. Now you can alt tab without crashing.

4gbextender. Use it. Love it. You need it.
http://newvegas.nexusmods.com/mods/35262
Casino heist. Adds lots of caps to the casinos since they didn't have any for you to rob.

Arwens realism tweaks is one of those acquired tastes. It makes so many changes to the game, it's like a whole new game at times. You have to have a knife to skin stuff, you must worry about bacteria infections, strain from carrying too much gear (or you will pass out), and if one of your limbs gets crippled you can't use two handed weapons at all. So many changes that I can't list them all. Just read it to see if you want it.

The script extender is required by a number of mods. In the description info they will let you know. Watch these tutorials. I can help you if you need it too. Installing mods for New Vegas can be frustrating for beginners, but once you learn the ropes, it's actually pretty simple. Sometimes it depends on the mod, but usually, with Nexus Mod Manager, you don't have to do much of anything besides click a few buttons. The script extender isn't always required, it just depends on what mods you want to use. Project Nevada? You need it. 4g extender? Need it. CASM (save manager)? Need it. Basically, the most popular mods need it.

It's not a bad thing to install at any rate. Drag some files into the New Vegas folder. Done.
I'm currently running these mods. Edit: Now I've added a bunch more but this load order was very stable..

Oh I removed IWS though. Way too unstable. My only complaint with NMM is that I had to manually type that list. I added AWOP, Monster mod version 10, and associated patches. Pretty stable so far, 40+ hours in. I just can't get enough of this game. Playing vanilla is out of the question for me though. I've played vanilla waaaay too many times, in depth, all quest variations, all weapons, all endings, just nothing new to do that way. It would be like playing Fallout 2 without the restoration patch. Near heresy round these parts.
AWOP does have some fairly stupid shit in it, but the level design trumps that stuff. I made a mod to fix some of the annoying stuff.
You need AWOP of course, and I advise you to download the low loot and low npc patches as well.
