Doctor is more useful later on in the game when you can get the implants and as such I wouldn't tag it, I'd start to increase it after you've got your other three tag skills (Small Guns, Speech & Lockpick for me) to the 100+ mark.
After them the fourth most useful skill is probably Doctor (or Big Guns/ Energy weapons if you want some heavy firepower for the end game). So as for the eye thing, I think you'll have to bite the bullet and spend some of those points on your Doctor skill.
