Time to shut my yap on this one. It`s getting too frustrating just thinking that a fan would do this to Fallout.
It wasn't Gordian who cut the knot, it was Gordian who tied it. Twas Alexander who cut it.
With that historical misstep out of the way i think you could pull off the wastelands along with the cities. First it couldn't have the world map. Second it couldn't have one continuous map. The best idea is to have a mass transit system of sorts. The caravans would be the intercity transportation for new characters and eventually you could buy a car, but then you've got fuel
Now before the transit starts it will calculate if you'll have an encounter if so, how many. Naturally there are more dangerous caravans(high pay) that have more encounters and an almost completely safe method(little pay). The real trouble comes in with the First Aid and Doctor skills, along with the books and other similar things. Which I suppose could all be fixed through extensive scripting.
Most, if not ALL of the quests would have to run in an instance to preserve the "hero" aspect of a character. Or the other way, your just a normal guy(or gal) trying to make it in the wastes. if you want to be a hero, you've got to be one yourself.
And of course with the avid fanbase that Fallout has, you could allow players to create player factions(instead of guilds) that would then issue quests to faction members and faction hopefuls, even faction wars complete with, allies, merc contracts, money making operations, bases, and NPC's (hired through special NPC's) that would be the way to do FOOL. Have all the cool stuff player driven, I could easily see a Neo-Enclave or something of a sort.
